Liability Waiver and Release

I understand that Youth Power and Performance (YPP) is a physical training program that includes, but is not limited to, running, sprinting, jumping, change of direction work, strength and power exercises, and other athletic drills that can place stress on muscles, joints, bones, and the cardiovascular system.

I acknowledge that participation in athletic activity carries inherent risks, including, but not limited to, muscle strains, sprains, joint injuries, falls, collisions, impact injuries, overuse injuries, and in rare cases more serious events such as heat illness or cardiac events. I understand that YPP coaches and staff will take reasonable steps to provide a safe training environment but cannot eliminate all risk.

In consideration of my child’s participation in YPP, I voluntarily assume all such risks on behalf of my child. I agree that Youth Power and Performance, its owners, coaches, staff, agents, and facility partners will not be held liable for any injury, accident, illness, or health issue that may occur during, or as a result of, normal training activities, except in cases of gross negligence or intentional misconduct.

I agree to release, indemnify, and hold harmless Youth Power and Performance, its owners, coaches, staff, agents, and facility partners from any and all claims, demands, causes of action, damages, or liabilities arising out of or related to my child’s participation in the program, including reasonable attorney’s fees, to the fullest extent permitted by law.

I certify that I have disclosed any relevant medical conditions, limitations, or injuries in this form and that my child has permission from a parent or legal guardian to participate in athletic training. I understand that YPP does not provide medical advice and that it is my responsibility to seek clearance from a medical professional if I have any concerns about my child’s ability to safely participate.

I understand that YPP does not guarantee specific performance results, college scholarships, team placements, or playing time outcomes. Training is designed to improve athletic qualities, but results may vary by individual.

In the event of an emergency where I cannot be reached, I authorize YPP staff to seek or arrange emergency medical care for my child and I accept full responsibility for any associated costs. I confirm that I am financially responsible for my child’s medical care.

Media Release

This section is optional in terms of permission choice, but parents must select Yes or No.

Program Policies

By registering an athlete for the free YPP Performance Testing Day and for any future YPP training services purchased for that athlete, I understand and agree to the following:

  1. This registration is for a free Performance Testing Day. It does not begin a paid membership, create an automatic renewal, or authorize a charge.
  2. Performance Testing Day participation is subject to event capacity. Each participating athlete must be included on this registration.
  3. If an athlete cannot attend Performance Testing Day, the parent should notify YPP as soon as possible so the available spot can be offered to another athlete.
  4. If YPP changes or cancels Performance Testing Day because of weather, field conditions, safety concerns, or another operational issue, families will be notified using the contact information provided.
  5. Performance testing results represent the athlete's performance on the testing date. Results are not a medical evaluation and do not guarantee future athletic performance, team placement, playing time, or scholarships.
  6. If a monthly membership is purchased in the future, it will be prepaid and will automatically renew every 30 days from the original enrollment date unless canceled before the next billing date.
  7. Future monthly memberships may be paused, upgraded, downgraded, or canceled through the Parent Portal. Changes must be completed before the next billing date to affect the upcoming billing period.
  8. If a two month upfront package is purchased in the future, it will be prepaid, will not automatically renew, and will remain active for the full two month package period. Parents decide whether and when to purchase another package.
  9. All future YPP purchases and payments are nonrefundable. Parents are responsible for reviewing the applicable program details, schedules, pricing, and policies before completing a purchase.
  10. Future training sessions must be booked through the Parent Portal at least one hour before the scheduled start time. Athletes should not attend without a confirmed booking.
  11. Future training sessions must be canceled through the Parent Portal at least one hour before the scheduled start time. When canceled on time, the session credit remains available for future use within the applicable credit usage period.
  12. Late cancellations and no shows for future training sessions result in the loss of the session credit. Missed sessions are not refunded.
  13. Unused future training credits may be used at any time during an active monthly membership or upfront package period, subject to session availability.
  14. If a future monthly membership ends, any remaining credits may be used for up to four additional weeks after the final prepaid billing period ends.
  15. Future two month upfront package holders may use their credits throughout the full two month package period and for up to four additional weeks after the package ends.
  16. Future training credits cannot be refunded, transferred to another athlete, exchanged for cash, or extended beyond the applicable usage period without written approval from YPP.
  17. If YPP cancels a future training session, including because of weather, the session credit is returned. Weather updates and cancellation announcements are communicated by email and posted in the Parent Portal. Final decisions are typically made approximately 30 minutes before training.
  18. Eligible siblings receive a 15 percent discount on the second and each additional athlete's qualifying membership. Each athlete must be registered separately. Sibling discounts do not apply to drop in sessions.
  19. Athletes must follow coaching instructions, respect coaches and other athletes, and behave safely during Performance Testing Day and any future YPP training. Unsafe, disrespectful, or repeatedly disruptive behavior may result in removal from the event, a session, or the program without a refund.
  20. Parents must inform YPP of any injury, illness, health change, medication, or physical restriction that could affect their athlete's safe participation.
  21. Parents are responsible for ensuring safe and timely drop off and pickup. YPP is responsible for athletes only during the scheduled event or training session and is not responsible for athletes before or after scheduled participation times.
