I understand that Youth Power and Performance (YPP) is a physical training program that includes, but is not limited to, running, sprinting, jumping, change of direction work, strength and power exercises, and other athletic drills that can place stress on muscles, joints, bones, and the cardiovascular system.

I acknowledge that participation in athletic activity carries inherent risks, including, but not limited to, muscle strains, sprains, joint injuries, falls, collisions, impact injuries, overuse injuries, and in rare cases more serious events such as heat illness or cardiac events. I understand that YPP coaches and staff will take reasonable steps to provide a safe training environment but cannot eliminate all risk.

I agree that Youth Power and Performance, its owners, coaches, staff, agents, and facility partners will not be held liable for any injury, accident, illness, or health issue that may occur during, or as a result of, normal training activities, except in cases of gross negligence or intentional misconduct.

I agree to release, indemnify, and hold harmless Youth Power and Performance, its owners, coaches, staff, agents, and facility partners from any and all claims, demands, causes of action, damages, or liabilities arising out of or related to my child’s participation in the program, including reasonable attorney’s fees, to the fullest extent permitted by law.

I certify that I have disclosed any relevant medical conditions, limitations, or injuries in this form and that my child has permission from a parent or legal guardian to participate in athletic training. I understand that YPP does not provide medical advice and that it is my responsibility to seek clearance from a medical professional if I have any concerns about my child’s ability to safely participate.
